This offline channel is vital for persons with disabilities who have limited internet access, do not use smartphones, or face issues with population data integration, which prevents their card status from being active in the central system.

Through Minister of Social Affairs Regulation (Permensos) Number 2 of 2021, the government has established systematic technical guidelines so that the process of applying for your physical card and national data can be handled directly at the village or sub-district level. Below is a comprehensive guide covering the requirements, form completion procedures, and the offline KPD registration flow.

Who Can Register Offline?

Based on Article 5 paragraph (1) of Permensos Number 2021, offline registration is intended for:

  • Persons with disabilities who are not yet registered at all in the National Disability Data (DNPD) or National Socio-Economic Single Data (DTSEN).
  • KPD holders who wish to report changes in personal data (such as change of domicile address, change of employment status, or update of disability type).

Collective or Represented by Power of Attorney

For persons with severe disabilities or those with severe mobility impairments that prevent them from visiting the sub-district office directly, the registration process does not need to be forced. In accordance with Article 9 of Permensos 2/2021, registration can be assisted and represented by another party by attaching a Power of Attorney or an official Assignment Letter. Authorized representatives of the applicant include:

  1. Blood relatives or legal guardians.
  2. Local village officials or sub-district officers.
  3. Social Workers (Peksos) or Social Welfare Personnel (TKS).
  4. Disability Organizations (OPDIS) under which the applicant is affiliated.

Required Document Files to Prepare

Before visiting the local sub-district or village office, ensure you have prepared the following administrative documents completely to avoid rejection:

  • KPD Registration Form I: The official form provided free of charge at the village/sub-district office or can be self-printed from the ministerial regulation's appendix.
  • Photocopy of Population Documents: Consisting of a photocopy of the applicant's active Identity Card (KTP) and/or Family Card (KK).
  • Disability Certificate: An official certificate signed by a local health official (healthcare facility/Puskesmas/Hospital) or Social Worker (Peksos). This letter must state the type of disability and the level/degree of the applicant's disability.
  • Recent Passport Photos: Prepare 2 recent passport-sized photos (2x3 cm).

Additional Note: If you are registering due to data changes, you must bring and attach your old physical KPD.


Guide to Filling Out KPD Registration Form I

In accordance with the official instructions for filling out KPD Form I in Appendix I of Permensos Number 2 of 2021, data entry must comply with the following writing rules:

  1. The form must be filled out manually using block letters (capital) and with black ink.
  2. For disability type or application type selection columns, mark an 'X' in the provided box.
  3. Write the applicant's full name as it appears on their birth certificate or official population documents, without including academic titles, religious titles, or noble titles.
  4. Affix a 2x3 cm passport photo in the designated column, then place the applicant's signature or thumbprint within the specified boundary lines.
  5. Once fully completed, the form must be signed by the applicant and acknowledged by the local Head of Village/Sub-district and Sub-district Head.

Step-by-Step Offline Registration Flow

The offline data verification process proceeds in stages from the neighborhood level to final verification by the central Ministry of Social Affairs:

Step 1: Document Submission at the Sub-district Office

The applicant or a representative with a power of attorney submits KPD Form I along with all supporting documents to the social service officer or operator at the sub-district/village office. The officer will check the physical completeness of the documents.

Step 2: Data Update by SIKS-NG Operator

Village/sub-district officials will input the applicant's data into the SIKS-NG (Next-Generation Social Welfare Information System) application. To expedite KPD activation, ensure the sub-district operator uses the partial data update (KPD shortcut) feature. Through this shortcut, the operator only needs to input two main variables:

  1. The applicant's type of disability (Physical, Intellectual, Mental, or Sensory).
  2. The level or degree of disability (Mild, Moderate, or Severe).

This quick step eliminates the need to fill in up to 40 general welfare data variables, significantly shortening the verification process for your disability status in the central system.

Step 3: Staged Verification and Endorsement Process

After being input at the sub-district level via SIKS-NG, the proposed data will be processed in stages for verification and validation by the regent/mayor through the sub-district head, then forwarded to the governor, and finally reported to the Minister of Social Affairs at the central level. The Minister of Social Affairs will then establish the applicant's data in the Decree of National Disability Data (SK DNPD).

Step 4: Card Printing and Distribution

Once the SK DNPD is signed by the Minister of Social Affairs, your Virtual KPD is automatically active and accessible. For those who desire a physical polymer plastic card, the Ministry of Social Affairs will print the KPD and distribute it in stages:

Central Ministry of Social Affairs → District/City Social Service → Sub-district Office → Village/Sub-district Office → Directly handed over to the Person with Disability as the rightful owner.

Addressing Service Issues in Regions

If, during implementation, village, sub-district, or local Social Service officials are unresponsive, do not understand PP 31/2026 procedures, or reject your registration process, you or your accompanying OPDIS can file a direct complaint to the central government through the following official Ministry of Social Affairs channels:

  • Ministry of Social Affairs Call Center: Call 171 (24-hour open complaint service).
  • Kemensos WhatsApp Center: Send a written complaint message along with proof of the issue to 0887-7171-171.
  • Kemensos Sentra: You can also visit the nearest service unit or one of the Ministry of Social Affairs' rehabilitation Sentra offices located in your area for assistance with data advocacy.
